This cancellation request form is utilized as a customer service tool to assure effective, accurate and clear communication that is to be managed through the appropriate business operations department. All cancellation requests are managed through the business operations office via the cancellation request form. Employees, Instructors, Program Directors and/or managers are not permitted to discuss or manage client cancellation requests as the form is managed through the business operations office.

Please note: The membership agreement is non-cancellable other than the “Rights to cancel” within the membership agreement seen below.

Rights to cancellation:

This membership agreement is cancellable with acceptable academy completed document and documents of proof of the students move more than twenty miles from an affiliated location, once the move has occurred. Examples: verification from school system child is enrolled in, utility bill in name on agreement, driver’s license with current address. Academy has the right to request proper documentation in writing. An appropriate administrative and legal fee of 299.00 will apply to all cancellations and must be paid by certified check prior to cancelling the agreement. Advanced payment on paid in full accounts revert back to the monthly price.

Please allow 6-8 weeks for processing after the acceptance of cause and receipt of cancellation payment. Agreement by both parties must be signed as acceptance of cancellation. All communication will be via U.S. Mail service.


I understand that I have signed a contract/note of my own will, which may be purchased by a financial institution. My failure and/or students to regularly attend and utilize center facilities does not relieve me (the buyer(s)) of my obligations set forth in this Membership Agreement, regardless of the circumstances. If this account is referred to an attorney for collection then the undersigned person (s) promise and agree to pay all collection costs including attorney fees of 33 1/3% of the principal amount due and owing when turned over for collection and do further agree to pay interest at a rate of 1 ½% per month (18% per Annum) on the unpaid balance from the date the services were last rendered. I authorize photocopies of this form to be as valid as the original. In the event this matter is turned over for collection, I hereby expressly give permission for my current employer (s) to provide verification of my said employment.

Buyer(s) Acknowledgement

I the buyer(s) agree not to harass, coerce, discuss cancellations, or financial concerns with the instructors and/or the programs directors.

All communication with regard to a cancellation request that is not a right to cancel with regard to the membership agreement shall be placed in writing and sent to 7410 Preston Road, Suite 105 Frisco, Texas 75034 and will be forwarded to the legal department to be documented.
