Client Has Moved

I hereby truthfully acknowledge that my request to cancel my membership cannot be reviewed and/or processed until all the required documentation and payment has been received after I have moved. I also understand that if all the criteria are not met, the cancellation of the membership will not be processed. As per the membership(s) contractual agreement, memberships are non­cancellable other than stated, marked, or typed therein. Any such request to cancel said membership in partial or in whole, requires a non- refundable administration review fee of $65.00 (sixty five dollars) to be paid prior to review and consideration.

Payment is to be mailed to 7410 Preston Road, Suite 105 Frisco, Texas 75034 after this cancellation form has been submitted on the website. Any such request that meets the required criteria for cancellation, the $65.00 administration fee will be applied to the membership(s) cancellation fee. A response will be sent within 7-14 days upon receiving this cancellation request, payment and any such required documents needed for review. Any additional documentation needed may require a longer time frame for a cancellation response as reasons for requested cancellation may vary. I hereby state that all information that I have submitted on this form to be the truth.
